The European certificate of origin form comprises:
The applicant (i.e. the exporter) must sign this form. The authorised chamber signatory will also stamp and sign this copy with the date of issue of the certificate of origin. Details of any supporting documents presented must also be included as well as the numbers of copies on the back of this pink copy. This pink application form and other documents relating to the issue of certificates of origin will be retained in a secure place by Athlone Chamber for at least three years from the date on which the Certificate of Origin is issued (EC Regulation n 2913/92 – art.16)
- An original form and 2 yellow copies
After checking and authenticating these documents, Athlone Chamber will stamp, sign and date each copy and return to the applicant.
Athlone Chamber will keep the pink copy for our own records and hand back the original and 2 yellow copies. Only one original certificate of origin will be issued for each consignment. Where trade reasons require, additional copies of each certificate of origin may be issued.
Advice on completion of applications for EU Certificates of Origin
The Consignor must be based within the 26 Counties of the Republic of Ireland
Name and address of the company must be typed in the consignor box, as must:
The consignee in the company to which the goods are being exported
The country of origin (box 3) is the country where the goods were manufactures. Please note the comments on the rear of the form on this respect.
The transport details are optional (box 4).
The remark details are also optional to be completed if under a letter of credit.
Box number 6 is to contain the description of the consignment.
Box number 7 is the quantity column.
When all details are completed by the applicant a line must be drawn underneath, so no more information can be added after it is certifies.
The applicant must also type place and date of issue.
All duplicate sheets of the form should be kept together and the application must be signed on the final page.
Please return the certificate (all copies) to Athlone Chamber of Commerce for signing and stamping.
